When to use it

  • The day it happens — never the week after. Memory is unreliable in legal proceedings.
  • Even for incidents that resolve on the spot — the paper trail is the protection.
  • Anything involving an injury, a slip, an allergen reaction, or a customer-staff altercation.
Common mistakes

What operators get wrong

  • Filling it out from memory days later. Time, exact words, witnesses — all degrade fast.
  • Skipping the witness signatures. A witness who confirms today is hard to call back next year.
  • Not photographing the scene. The phone in your pocket is part of the report.
